Our team connects families with trusted in-home care, mental health support, and community-based services—often introducing people to resources they never knew existed.

The Safeway Outreach Department connects Minnesotans to vital services designed to help individuals and families live safely, independently, and with dignity.

1. Mental Health Services

Our outreach team works closely with individuals and families to connect them to culturally responsive mental health care. Many people struggle in silence due to language barriers, stigma, or lack of information. By partnering with Minnesota’s county-based programs and licensed providers, we help people access resources that promote healing and long-term well-being.

2. Home and Community Support Services

We understand that care extends far beyond medical needs. Our outreach team helps families connect with in-home and community-based supports such as personal care assistance (PCA) and homemaking services. These resources ensure that individuals with disabilities, mental health challenges, or other complex needs can remain safely in their homes and communities.

3. Education and Navigation of County-Based Resources

Navigating county-based care systems can feel overwhelming—especially for families new to Minnesota or unfamiliar with how public programs work. The Outreach Department provides hands-on guidance to help people understand their options, complete necessary paperwork, and connect with the right county partners.
