What We Offer
We provide a range of services funded through Minnesota’s public programs. Each one is designed to support whole-person care—meeting both practical and emotional needs.
Adult Rehabilitative Mental Health Services (ARMHS)
Support for adults managing mental health diagnoses who want to build skills and confidence to live independently. We focus on whole-person progress, emotional stability, and real-world skills like scheduling, communication, conflict resolution, and coping strategies.
- Care is flexible, goal-driven, and community-based
- Delivered by Mental Health Practitioners or Mental Health Professionals
- Available in-home or in the community
- Ideal for individuals transitioning out of hospitalization or into more independent living
Housing Stabilization Services (HSS)
Housing-focused services that help individuals find and maintain safe, stable homes. Great for those at risk of homelessness or facing housing transitions.
- Housing consultation, transition, and sustaining services
- Support with applications, budgeting, landlord communication, and more
- Designed for individuals with disabilities, mental health conditions, or other complex barriers
245D Waiver Services
Individualized Home Supports and community-based support for people with disabilities, funded through Minnesota’s waiver programs.
- Services include assistance with daily living, medication reminders, meal prep, and more
- Delivered in-home or in the community
- Customized based on the person’s goals and support plan
- Aimed at promoting independence and community inclusion
(CFSS) / Personal Support
Hands-on daily living assistance for people who need help with physical tasks like bathing, dressing, eating, or mobility.
- Available for both children and adults
- Often included as part of a broader support plan
- Helps individuals remain safe, comfortable, and supported at home
